Southwest District Health will be holding a Flu Shot Clinic on Wednesday November 3rd from 9am-2pm in the HUB. Cost is only $35 if you do not insurance. Please fill out form below and bring with you or you can pick up a form at the District office Monday-Thursday 7:30am- 4:30pm.
